The Body Access Coordinator is responsible for facilitating appropriate body access assessment and maintenance for CKD patients, hemodialysis (HD) patients, and peritoneal dialysis (PD) patients as well as supporting the learning needs of dialysis nurses related to body access. Based on the needs of the Kidney/Urinary program, the body access nurse is responsible for:

  • Current registration with the College of Nurses of Ontario required
  • Minimum 3 years hemodialysis experience required
  • Current BCLS/HCP certification required
  • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families
  • Gentle Persuasive Approach training preferred
  • Demonstrated hemodialysis vascular access competency required, including knowledge of vascular anatomy, ultrasound guided cannulation skills and ability to assess access for complications required
  • Demonstrated excellence in communication and documentation skills required
  • Demonstrated commitment to ongoing professional development required
  • Minimum two years of general surgical/medical nursing preferred
  • C Neph certification preferred
  • Hemodialysis Access Resource Team would be an asset
  • Continuing education courses in physical assessment, adult education or communication skills would be an asset
  • Willingness and ability to work regular or extended tours as required in order to meet the needs of the clinic
  • A good attendance and work record demonstrating reliability and dependability is essential
  • A resume indicating further education, special courses and experience should accompany the application

  • Ensuring processes, resources and tools are in place to ensure patients and their families can receive consistent and high quality dialysis access-related education and support in making modality decisions. This includes patients in predialysis care, existing dialysis patients and patients transitioning from transplant to dialysis
  • Ensuring each patient has been assessed for the most appropriate dialysis access
  • Maintaining open communication with each Nephrologist to review patients and discuss access options and create plans of care
  • Directing, coordinating, and triaging access related care of CKD and hemodialysis patients between nephrology, surgery and interventional radiology
  • Leading HART (hemodialysis access resource team) team and other staff access education initiatives
  • Collecting and reporting data for the Ontario Renal Network and program quality improvement initiatives
  • Supporting the clinic activity for body access assessment and follow-up
  • Participation in provincial and program quality improvement initiatives
  • Facilitating accurate data reporting related to body access
