Job Description

Job Summary

The Social Worker: This full-time position is responsible for intake coordination, assessment of new members, and one-on-one sessions with members. Key to this position is a client-centered approach and the development of supportive relationships which will be helpful in guiding clients through their options. In addition, the social worker will facilitate virtual and in-person group programs offered by the CPA.

We are looking for a team member who is knowledgeable about the disability sector in Calgary and is able to help persons with disabilities navigate various systems. In addition, this person would act as an advocate and help persons with disabilities foster their capacities and coping mechanisms. Among others, the ideal candidate is one who is not afraid to start new initiatives, is practical and flexible, and possesses good observational skills.

Responsibilities:

Client Intake and Support

· First point of contact for clients wishing to join the CPA as a client/member

· Responsible for entering all client information in client/case management database

· Meet with clients for initial needs-assessment and to determine appropriate programs, services, referrals

· Provide counseling services to individuals with disabilities and their families, and offer referrals to appropriate resources, programs, or services, internally and externally

· Provide general information/education about CP to individuals and families

· Collect client profiles for needs-analysis and statistical information

· Annual check-in with CPA clients/members

· Conduct home or hospital visits when necessary

Program Facilitation and Mental Health

· Design and facilitate weekly social, educational, and life-skills workshops for 18+ groups

· Run weekly parent support group sessions and find guest speakers for this group

· Provide mental health resources to groups and design mental health tip-sheets with emerging mental health strategies

System Navigation

· Assist families who have just received a diagnosis for their child

· Facilitate the transition of individuals/families from youth to adult as well as adult to senior programs/services/supports

· Conduct/update environmental scans for housing, employment, funding etc. for people with disabilities

Community Representation

· Encourage all members to become more active in a wide range of activities aimed at improving overall quality of life

· Partake/ present, on behalf of the members of the CPA, at various public events to increase awareness of the “abilities” of CPAA members

· Coordinate with other team members to ensure consistency of information provided to clients/members

Job Type / Category

This is a full-time role, 37.5 hrs. /week

Required Education, Skills and Qualifications

· BSW required; other related disciplines and work experience will be considered in order to successfully fill the position

· Experience with intake, service coordination and/or scheduling processes

· Knowledge of community resources

· Knowledge of and proficiency in use of word processing, scheduling and database software

· Ability to organize & prioritize work, and to manage multiple projects at a time

· Advanced written and oral communications skills
